The Canadian Hospice Palliative Care Association and its partners will be hosting a “Learning Institute" focused on intermediate to advanced learning. The Institute will take place June 1, 2 and 3 at the Banff Centre in Banff Alberta.

** Students must to show proof of full time enrolment. (Fax, Photocopy or Scan)
*** Volunteer Definition: Volunteers are workers who have been officially screened and approved as a hospice palliative care volunteer within a hospice palliative care program or service. Volunteers give freely of their time and can provide written evidence of their position from their supervisor, manager or board of directors. Volunteers must provide a letter of reference (fax, photocopy or scan) from the organization for which they volunteer.

Cancellation Policy:
• On or before May 3rd, 2012 - Full refund minus a $75 administration fee
• May 3rd to June 3rd, 2012 - 50% refund minus a $75 administration fee
• On or after June 3rd, 2012 - No refund
Please note that there is no charge for transferring a registration to another person.
If your registration fee was paid by credit card the credit card will be refunded. If your registration paid was paid by cheque or money order, you will receive a refund cheque in the mail.
Prior to June 3rd, 2012, refunds will be processed within 30 days of receiving the request. All refund request received after June 3rd will be not be processed before August 3rd, 2012.
